Description
The objective of this proposal is to develop an web-based framework for the support, surveillance, clinical- and self-management of cancer survivors aged 12-29. The proposed support framework will serve as a decision and communication aid to enable both health professionals and survivors to make informed, effective and patient-sensitive decisions.The framework will improve the surveillance of young cancer survivors in their home-setting and allow the health professionals to monitor their progress and respond in time with effective interventions.
