this grant will enable the center to advance consumer partnership (cacp) to work with a range of organizations, including health plans, medicaid agencies, social services, and national nonprofits, to create sustained partnerships with the consumers and communities that those organizations serve. deliverables will include an equity framework for cacp's consumer partnership model; a refined, second generation of the consumer-journey mapping that cacp has created; and an explicitly focused equity model for consumer and community engagement.
