Description

First Link is an innovative referral program designed for people newly diagnosed with Alzheimer's disease and other dementias and their families. It is based on the philosophy that early access to ongoing services and support makes a huge difference to people with dementia, their families and caregivers. Evaluations of the program conducted in British Columbia, Saskatchewan, and Ontario demonstrate that it is has been successful at connecting individuals with dementia and their caregivers to services and education programs sooner than would have otherwise been the case. However, several questions about the program's effectiveness and impact still need to be addressed. Alzheimer Societies, as non-profit organizations with limited infrastructure, do not have the capacity to answer these questions on their own. The purpose of this planning initiative is to evaluate the First Link program, building on the strong relationships that Alzheimer Societies across the country have established with researchers and policy makers. A formal nationwide partnership will be created to focus on research that will inform the further development of First Link across Canada, influence related government policy, and contribute to knowledge on health system integration, dementia care, and program evaluation.
