Description
In Canada, 7% of the population takes 5 or more medications, including almost a third of seniors. As people's medication regimens become more complex, the risks of side effects, drug interactions and hospitalizations increases. It also makes it less likely that people will either choose to or be able to take their medications as prescribed. To support patients who take multiple medications, we need a primary care system and health records that help patients, pharmacists and primary care providers work together. Our objective is to support patient-engaged research on the current state of shared health records across patients, primary care clinics and pharmacies, including the types of information typically exchanged, the best practices and the tools that support information exchange.
