Description
The overarching goal is to examine how capturing key features of CBPHC performance relative to equity and outcomes for complex vulnerable patients can influence decisions on the design and delivery of CBPHC to improve care for these patients, leading to a stronger system for all Canadians. To achieve this we will conduct four interrelated studies in British Columbia, Ontario, and Quebec, build research capacity and engage in effective knowledge exchange: a. Implement the intervention of a performance measurement system that will operationalize equity-sensitive, pan-Canadian CBPHC structure, process, and outcome indicators (Study A); b. Identify the organizational and delivery attributes necessary for scaling-up CBPHC models aimed at achieving improved health outcomes and equity for complex vulnerable populations (Study B); c. Evaluate policy and funding contexts using a social justice and equity framework to determine the conditions necessary to scale-up equity-oriented innovative CBPHC models (Study C); d. Implement a reporting system intervention, using the CBPHC indicators, as a strategy to scale-up innovative models of CBPHC (Study D) Two objectives related to building capacity and knowledge exchange are: 1. To build on and expand interprofessional and interdisciplinary research capacity involving collaborations among university based researchers, research trainees and decision-makers as knowledge users and decision-makers with a view to promoting health equity within CBPHC delivery. 2. To engage in effective knowledge exchange to enhance integration and uptake of a CBPHC system-level performance measurement and reporting system that will provide the evidence base to improve decision making and guide transformative change of health care delivery in Canada.
