We studied 1,664 consecutive patients hospitalized with acute myocardial infarction (MI) between 1997 and 1998 to determine the association between blood glucose level at admission and mortality in the contemporary era. Regardless of admission blood glucose level, patients with diabetes had a worse outcome than patients not known to be diabetic whose admission blood glucose was normal. Patients without diabetes who were hyperglycemic on admission had the worst in-hospital outcomes and the highest mortality out to 1 year. Blood glucose remains a powerful independent predictor of poor outcome in acute MI.
